DMV Fax Number: How to Fax the DMV (by State)

There is no single DMV fax number: every state runs its own agency, and each unit inside it has its own fax line. Here are the DMV fax numbers for 14 states, taken from each agency's own pages and forms, what each line accepts, and how to fax the DMV from your phone.

DMV Fax Number: How to Fax the DMV (by State)

Quick answer: There is no single DMV fax number. Every state runs its own motor vehicle agency, and each unit inside it (medical review, insurance, CDL) has its own fax line. Fax the unit named on your DMV letter or form, and keep the delivery receipt. You can send it from your phone with mFax, free for 7 days.

Looking for a DMV fax number usually means a letter has arrived: a medical report is due, an insurance lapse needs clearing, or a CDL form has a deadline. The catch is that "the DMV" doesn't have one fax line. In Florida, Georgia and New Jersey the agency isn't even called a DMV, and in Texas the DMV doesn't issue driver licenses.

Below are the fax numbers for 14 states, each taken from the agency's own website or form and labeled with the unit it reaches and what it accepts. To send without a fax machine, mFax turns a photo or PDF into a fax from your phone or browser. It hands you a PDF receipt once the DMV's machine confirms every page.

Why There's No Single DMV Fax Number

Driver licensing is run by the states, and each state names its agency differently. Florida has the Department of Highway Safety and Motor Vehicles (FLHSMV), Georgia the Department of Driver Services (DDS), New Jersey the Motor Vehicle Commission (MVC), and Louisiana the Office of Motor Vehicles (OMV). Texas splits the job: driver licenses sit with the Department of Public Safety (DPS), vehicles with the Texas DMV.

Inside each agency, most fax lines belong to individual units, not to the agency as a whole. The medical review unit has one number, insurance has another, and the CDL desk a third. A number copied onto a directory site rarely says which unit it reaches, and a DMV phone number is not a fax line: send a fax to a voice line and mFax reports No fax tone or Declined.

The notes below give the address, the phone line and the source for each number, and the cases where a fax isn't the right route.

California (DMV)

California's DMV lists no fax number for the Mandatory Actions Unit. The unit takes the Application for Termination of Action (DL 4006) by email at DMVLODMAU1650@dmv.ca.gov or by mail to M/S J233, PO Box 942890, Sacramento, CA 94290-0001, and answers questions at (916) 657-6525.

The fax lines the DMV does list are for paperwork most drivers never send. Bulk requests for more than 15 records go to the Materials Management Section at (916) 928-6817, and Public Records Act requests to the Legal Affairs Division at (916) 657-6243 (Records Requests). The DMV also treats a fax copy exactly like a photocopy: a fax is accepted only where a photocopy is, and a copy of a title never is (registration procedures manual).

Delaware (DMV)

CDL holders fax the self-certification affidavit to (302) 739-2602, attention CDL Department, or email it to DOT_DECDLMEDCERT@state.de.us. The medical certificate itself no longer goes through you: after June 23, 2025, Delaware accepts it only electronically through FMCSA's National Registry (CDL page).

Proof of insurance has a fax line at each DMV location: Wilmington (302) 577-5655, Delaware City (302) 834-2641, Dover (302) 744-2574 and Georgetown (302) 853-1038. Only your insurance company may use them; the DMV states that customer faxes will not be accepted. Ask your insurer to fax it on the day of your visit, to the location you'll visit, showing the vehicle's make, year and VIN, the insurer's NAIC code, the insured's name and the policy dates (insurance fax numbers).

Florida (FLHSMV, Tallahassee)

Florida's fax lines ring FLHSMV headquarters in Tallahassee. Medical reports go to the Medical Review Section at (850) 617-3944 (phone (850) 617-3814), and vision reports to the Vision Section at (850) 617-3936 (Medical Review contact page).

To get the other driver's insurance details after a crash, fax the Insurance Request Form (HSMV 83392) with a complete copy of the crash report to the Customer Service Correspondence Center at (850) 617-5216 (Involved in a Crash?). The form says to allow 10–15 business days, and the wait is the same by mail or fax.

Georgia (DDS)

Georgia's Medical Review Unit takes faxes at (770) 918-6266, the number in the instructions of both the Medical Report (DDS-MR 287, rev. 02/2026) and the Request for Medical Review (DDS-270). The DDS-270 also prints a second line, (770) 344-3629, in its header.

The medical report has a catch: your medical provider must mail or fax it directly, with a cover sheet and a copy of the DDS letter. You don't send it yourself (DDS-MR 287). The mailing address is PO Box 80447, Conyers, GA 30013.

Louisiana (OMV)

Louisiana's Office of Motor Vehicles lists (225) 925-3901 for CDL medical examiner certificates, alongside email (OMV_CDL_Medical@dps.la.gov) and mail to P.O. Box 64886, Baton Rouge, LA 70896 (Medical & Vision Requirements).

Check before you send it. OMV says that starting September 15, 2026, CDL drivers no longer need to submit a copy of their DOT medical certificate, because medical providers report it to the Federal Motor Carrier Safety Administration instead (Submit CDL Medical Certificate).

Nevada (DMV)

Nevada's DMV takes the Confidential Physicians Report (DLD-007, rev. 10/2026) at (775) 684-4829, at its Carson City office, 555 Wright Way. The form warns that the DMV must receive it within 30 days of the examination (DLD-007).

New Jersey (MVC, Trenton)

New Jersey's Medical Review Unit is at (609) 292-7504 by fax and (609) 292-7500 ext. 5032 by phone, with mail to P.O. Box 173, Trenton, NJ 08666-0173 (Medical Review contact).

Proof of insurance for an uninsured-motorist suspension goes to (609) 777-3178 or (609) 777-3179, or by email to UM.INFO@MVC.NJ.GOV. Send a copy of your current NJ insurance ID card or the policy's declaration page, and put your NJ driver's license number and a return phone number on everything you send (Suspensions).

New York (DMV)

The NYS DMV Medical Review Unit's fax is (518) 402-2991. Its phone line is (518) 474-0774, option 2, weekdays 8:30 a.m. to 4:00 p.m., and the mailing address is 6 Empire State Plaza, Room 337, Albany, NY 12228 (Medical Review Program).

North Carolina (NCDMV)

The NCDMV Medical Review Unit's fax is (919) 733-9569 (phone (919) 861-3570 or (919) 861-3809). It takes seatbelt waivers, reconsideration requests and hearing appeals by fax or mail.

Two things must be mailed instead: removal requests (form DL-79 or a signed letter) and requests for your medical records, which need a notarized release and a $10 check or money order (Medical Review Program).

Oregon (DMV)

Oregon DMV's Driver Specialty Services in Salem takes medical referrals from health-care providers (form 735-7230) and driver evaluation requests (form 735-6066) at (503) 945-5329, or by email at DMVDriverSafetyTeam@odot.oregon.gov. Form 735-7230 asks senders to call (503) 945-5083 to confirm the report arrived (form 735-7230).

Pennsylvania (PennDOT)

PennDOT's Medical Unit (Bureau of Driver Licensing, Driver Qualifications Section) takes medical reports at (717) 705-4415, or by mail to P.O. Box 68682, Harrisburg, PA 17106-8682 (Medical Reporting FAQs).

CDL holders can fax the self-certification form DL-11CD to (717) 783-5429, or email it to CDLmedscert@pa.gov, when it's fewer than 30 days before the due date; earlier than that, mail it. The medical examiner's certificate is different: PennDOT says it cannot be mailed or faxed and must arrive electronically from FMCSA's National Registry (Medical Examiner's Certification FAQs).

South Carolina (SCDMV, Blythewood)

SCDMV lists no general fax line for drivers. The fax number on its driver forms belongs to the CDL Help Desk: fax the self-certification form (DL-405A) and your medical documents to (803) 896-2676, or email them to CDLHelpDesk@dmv.sc.gov. Questions go to (803) 896-2673 (DL-405A).

Responses to an insurance notice (FR-4A/FR-31A) go by mail to the Financial Responsibility Office, PO Box 1498, Blythewood, SC 29016-0040. For anything else, call the SCDMV customer contact center at (803) 896-5000.

Texas (DPS, not TxDMV)

In Texas, driver licenses belong to the Department of Public Safety; the Texas Department of Motor Vehicles handles vehicles, not licenses. Medical information for the Medical Advisory Board, and written concerns about an unsafe driver, go to DPS Enforcement and Compliance Service at (512) 424-5311. You can also email MAB@dps.texas.gov or mail P.O. Box 4087, Austin, TX 78773-0320 (Texas Medical Evaluation Process).

Virginia (DMV, Richmond)

All three Virginia lines reach DMV headquarters in Richmond (P.O. Box 27412). Medical Review Services takes the MED 2 and MED 4 reports, which your medical provider mails or faxes, at (804) 367-1604. DMV asks for them 5–7 days before the suspension date on your letter (Medical Review of Drivers).

Virginia license or ID holders living out of state can fax address-exception documents to the DL/ID Card Work Center at (804) 367-1112, with their full name, license or ID number and date of birth (DMV 143). Police crash report requests go to (804) 367-0390 (Request a Police Crash Report).

Can You Fax Documents to the DMV?

Yes, for most of what a DMV unit asks you to send. Fax is how medical review units, insurance desks and CDL help desks expect their paperwork in many states. What you can't fax are originals, and a few lines only take faxes from a third party.

Usually accepted by fax:

  • Medical and vision reports a DMV letter asked for. In Georgia and Virginia your doctor sends them, not you
  • Proof of insurance for a suspension in New Jersey, or from your insurer in Delaware
  • CDL self-certification forms in Delaware, Pennsylvania and South Carolina
  • Seatbelt waivers, reconsideration requests and hearing appeals in North Carolina
  • Address-exception documents for Virginia license holders who live out of state

Not accepted by fax:

  • Titles. California's DMV treats a fax as a photocopy, and it doesn't accept a photocopy of a title.
  • The CDL medical examiner's certificate, in states that now take it only from FMCSA's National Registry. Pennsylvania and Delaware say so outright, and Louisiana no longer asks for a copy.
  • Legal papers. California's DMV does not consent to facsimile service of process (Service of Process).
  • Anything the unit wants by mail. North Carolina wants removal requests and medical-records requests by mail.

Use the number on your letter or form

A DMV phone number won't take a fax, and a number from a directory site may reach the wrong unit or a retired line. Use the fax number printed on your DMV letter or form. If neither has one, use the table above.

How to Fax the DMV From Your Phone

You don't need a fax machine or a trip to a print shop. The mFax app on iPhone, iPad and Android, or mFax in any browser, sends to a DMV fax line the same way it sends anywhere else. These steps use the app's own labels; our guide to faxing from an iPhone covers the phone side in more detail.

1

Find the unit's fax number

Take it from your DMV letter or from the form's instructions. Match the unit (medical review, insurance or CDL) to what you're sending.

2

Open mFax and choose New Fax

Open the app, or start in your browser, and choose New Fax. Enter the DMV's fax number with +1, the code for the US.

3

Add your pages and sign them

In the iPhone, iPad or Android app, scan the form with the camera (edge detection crops it into a clean PDF), or attach the PDF you already have. A fax can carry up to 60 pages and 100 MB. If the form needs your signature, open the page and choose Edit page, then Sign, or sign it first in our free PDF editor.

4

Add a cover sheet

Pick a template and fill in your name, the subject and a message: your license number, date of birth and the reference from the DMV letter. Tick Confidential to add a confidentiality notice to a medical report.

5

Send and keep the receipt

Check the page count and estimated send time, then press Send. The fax shows In progress while the pages cross, about 40 seconds a page, and Delivered once the DMV's machine confirms every page. Download receipt saves the proof as a PDF.

What to Put on a DMV Fax Cover Sheet

The cover sheet's job is to get your pages matched to your file. Put on it:

  • Your full name as it appears on your license
  • Your driver's license or ID number and date of birth (Virginia's DMV 143 asks for all three)
  • The reference or case number from your DMV letter
  • The unit you're sending to, for example "Medical Review Unit"
  • The page count and a return phone number (New Jersey asks for one)

Keep diagnoses off the cover sheet; the medical details belong on the form itself. Our medical fax cover sheet guide has a template that keeps the private part off the top page.

How Do You Know the DMV Got Your Fax?

The delivery receipt answers one question: did the DMV's machine receive every page. A fax that didn't bounce isn't proof of delivery. Delivered means the receiving terminal confirmed each page, and the PDF receipt shows it. That is what to keep when a deadline is involved. It confirms the transmission, not that a reviewer has opened your file; our page on fax confirmation explains what a receipt does and doesn't prove.

Processing is a separate clock. Florida's Insurance Request Form allows 10–15 business days. Virginia asks for medical documents 5–7 days before the suspension date. Oregon's forms ask you to call (503) 945-5083 to confirm a report arrived.

If a fax fails, the row says why:

  • Recipient busy: DMV fax lines take one call at a time. mFax retries busy lines automatically; otherwise wait a few minutes and press Resend.
  • No fax tone or Declined: the number answered, but no fax machine did. You have probably dialed a DMV phone line rather than a fax line.
  • Invalid number or Number not in service: check the digits against the form. Numbers copied onto directory sites can be out of date.

Finding the Fax Number for Another State

For a DMV fax number in a state not listed here, look in this order:

  1. Your DMV letter. It names the unit handling your case and usually how to reply.
  2. The form itself. Fax numbers are printed in the instructions of many DMV forms, such as Georgia's DDS-MR 287, Nevada's DLD-007 and Oregon's 735-7230.
  3. The agency's own website. Search the state's .gov site for the unit, for example "medical review" or "CDL medical certification", rather than for "fax".
  4. The main phone line. Ask which unit handles your document and what its fax number is. For example, SCDMV's is (803) 896-5000, NCDMV's (919) 715-7000 and PennDOT's 717-412-5300.

Federal agencies work the same way, one fax line per office or form. See our guides to the Social Security fax number, faxing the IRS and faxing government forms.

Send Your DMV Fax Now

Get mFax on the App Store or Google Play, or open it in your browser. Scan or upload the form, enter the DMV unit's fax number and send it free with the 7-day trial. The app is rated 4.7 on both stores, and you get a PDF receipt once the DMV's machine confirms every page.

Frequently Asked Questions

Can I fax documents to the DMV?
Yes, for most of the paperwork a DMV unit asks you for: medical reports, proof of insurance and CDL self-certification forms go by fax in many states, each to that unit's own number. Titles and other originals can't be faxed, and some lines only take faxes from your doctor or your insurer, so read the form's instructions first.
What is the NC DMV fax number?
The NCDMV Medical Review Unit's fax is (919) 733-9569. It takes seatbelt waivers, reconsideration requests and hearing appeals; removal requests and requests for your medical records go by mail.
What is the Oregon DMV fax number?
Oregon DMV's Driver Specialty Services takes medical referrals and driver evaluation requests at (503) 945-5329, or by email at DMVDriverSafetyTeam@odot.oregon.gov. Call (503) 945-5083 to confirm it arrived.
What is the fax number for the California DMV Mandatory Actions Unit?
The DMV lists no fax number for the Mandatory Actions Unit. Its Application for Termination of Action (DL 4006) goes by email to DMVLODMAU1650@dmv.ca.gov or by mail to M/S J233, PO Box 942890, Sacramento, CA 94290-0001, and questions go to (916) 657-6525.
Can I fax my CDL medical card to the DMV?
Usually not any more. Medical examiners now send CDL exam results to the state electronically through FMCSA's National Registry: Pennsylvania says the certificate "cannot be mailed or faxed", Delaware accepts it only electronically since June 23, 2025, and Louisiana no longer asks drivers for a copy as of September 15, 2026. The CDL self-certification form can still be faxed in states such as Pennsylvania, Delaware and South Carolina.
