Quick Answer: There is no single Anthem fax number — each state, each type of plan and each kind of request has its own line. Prior authorization for Anthem Medicare Advantage plans goes to 1-866-959-1537, Anthem Medicare members fax appeals and grievances to 1-888-458-1406 (medical) or 1-888-458-1407 (pharmacy), and Anthem's Medicaid plans in New York and Kentucky take medical prior authorization at 1-800-964-3627. A practice that faxes Anthem every week can send from its own number, with a signed BAA, on mFax Business.
Searching for an Anthem fax number usually turns up one state's contact page, and its numbers may not belong to your plan at all. Anthem is the Blue plan in 14 states — Anthem Blue Cross in California, Anthem Blue Cross and Blue Shield in the other 13 — and each state runs its own commercial, Medicaid and Medicare lines. Anthem wants most prior authorizations and appeals on Availity; fax is the fallback it prints for each program.
Below is every Anthem fax number we could confirm on Anthem's own pages and provider contact guides, read on October 26, 2026, with the document each one comes from. You'll also find where claims go instead of a fax line, what to put on the cover sheet, and how to fax patient records to Anthem with a business associate agreement in place.
Anthem Fax Numbers at a Glance

Your letter beats this table
If Anthem sent you a denial, a records request or an Explanation of Benefits, use the fax number or address printed on it. Check the ID card as well: Anthem is the Blue plan in its 14 states only, and in three of them only in part — Anthem's own site excludes Missouri's Kansas City area and the Northern Virginia suburbs of Washington, D.C., and lists 17 southeastern counties in New York. A patient covered by another state's Blue plan is handled through BlueCard, and Anthem's guides route that patient's preapproval to the home plan (1-800-676-2583).
Anthem Prior Authorization Fax Numbers
Anthem publishes prior authorization fax lines, but its contact guides send you to Availity first. Anthem's Interactive Care Reviewer (ICR) takes requests on Availity 24 hours a day: log in and choose Patient Registration › Authorizations & Referrals. Fax is the route Anthem keeps for when you can't, and each program prints its own number.
Two kinds of request skip Anthem's fax lines altogether. Services that Carelon Medical Benefits Management reviews for Anthem — advanced imaging, cardiology and sleep studies among them, with the exact list varying by state — go to Carelon instead, through providerportal.com or the Carelon phone line in your state's guide. Drugs on the pharmacy benefit go to the pharmacy lines further down.
Commercial and individual plans
Of the state contact guides we read, two print a commercial prior authorization fax:
| State and plans | Fax | Phone | Source |
|---|---|---|---|
| California (Anthem Blue Cross): local plans, CalPERS, National accounts | 1-866-815-0839 | 1-800-274-7767 (local) · 1-800-451-6780 (CalPERS) · 1-866-470-6244 (National) | Anthem Blue Cross provider contacts (CABC-CM-057323-24) |
| California: specialty drugs on the medical benefit | 1-866-408-7195 | 1-866-580-5293 | Same guide |
| Ohio: local plans and the Anthem Essential Network exchange plan | 1-800-266-3504 | 1-800-752-1182 | Ohio quick contact guide (OH-BCBS-CDCRCM-003626-25) |
The other guides send commercial requests to Availity or to a phone line. Indiana lists medical preapproval at 1-877-814-4803, New York lists Anthem's Medical Management Department at 1-800-441-2411, and Nevada points to the Interactive Care Reviewer or the preauthorization number on the member's ID card. If a reviewer asks for records on an open case, attach them in Availity or fax them to the number printed in the request.
Federal employees are their own case. The Indiana and Ohio guides list Federal Employee Program preapproval at 1-800-860-2156 and a prior authorization fax of 1-800-732-8318.
Medicare Advantage
Anthem's Medicare Advantage prior authorization fax is 1-866-959-1537, the one number that repeats from state to state:
- The Ohio guide lists it for acute inpatient notifications and all outpatient requests on individual plans (Anthem MediBlue Access, MediBlue Plus and MediBlue Dual Advantage).
- The New York and Nevada guides list it as the prior authorization fax, with 1-833-848-8730 as the phone line.
- Indiana uses it for its PathWays for Aging Dual Care plan.
Facility stays have a second line: the Ohio guide sends clinical information for skilled nursing, long-term acute care and inpatient rehab reviews to 1-877-423-9972.
Medicaid plans, state by state
Anthem's Medicaid plans publish the most fax numbers, and no two states match:
| State and plan | Medical prior authorization fax | Other fax lines | Provider phone |
|---|---|---|---|
| New York: Anthem Blue Cross and Blue Shield HP (Medicaid, Child Health Plus, HARP, MLTC, Essential Plan) | 1-800-964-3627 | Behavioral health inpatient 1-877-434-7578 · outpatient 1-866-877-5229 · retail pharmacy 1-844-490-4877 · medical injectables 1-844-493-9206 · MLTC provider fax 1-718-368-6267 | 1-800-450-8753 |

| Indiana: Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, PathWays for Aging | 1-866-406-2803 (physical health, inpatient and outpatient) | Concurrent review 1-844-765-5156 · outpatient clinical records 1-844-765-5157 · behavioral health (PathWays) 1-877-410-0623 | 1-866-408-6132 (HHW) · 1-844-533-1995 (HIP) · 1-844-284-1798 (HCC) · 1-833-569-4739 (PathWays) |
| Virginia: Anthem HealthKeepers Plus | Provider Services fax 1-866-408-7087 | Pharmacy prior authorization: retail 1-844-512-7020 · medical injectables 1-844-512-7022 | 1-800-901-0020 |
| Wisconsin: Anthem Medicaid and Family Care | Provider Services fax 1-800-964-3627 | — | 1-855-558-1443 (Medicaid) · 1-844-483-2704 (Family Care) |
Sources: each state's provider contact page on anthem.com, Indiana's precertification requirements page (updated October 17, 2025) and Virginia's pharmacy information page. Ohio and Nevada Medicaid print no prior authorization fax: both name Availity as the method, with 1-844-912-1226 (Ohio) and 1-844-396-2330 (Nevada) for questions.
Prescription drugs
For drugs on the pharmacy benefit, Anthem's commercial pharmacy page calls electronic prior authorization — through Surescripts, CoverMyMeds or your EHR — the preferred method. To fax instead, the number depends on who manages the member's drug coverage:
- CarelonRx, the pharmacy benefit manager Anthem uses, lists fax 1-844-521-6940 for prior authorization, prescription exceptions and utilization management, and 1-833-293-0659 for questions, on its contact page. If the drug's own prior authorization form prints a different fax, use the form's.
- Medicaid plans run their own pharmacy lines: New York, Kentucky and Virginia are in the table above.
If the member's ID card names another pharmacy benefit manager, such as CVS Caremark or Express Scripts, the drug request goes there instead: see our guides to the CVS Caremark fax number and the Express Scripts fax number.
Anthem Appeal Fax Numbers
The Anthem appeal fax number depends on who is appealing. Provider appeals mostly go through Availity or by mail, while Anthem's Medicare and Medicaid plans publish fax lines for members.
For providers
Anthem's contact guides put provider disputes on Availity first, where claim appeals and the Payment Appeal Tool live. On paper, each plan has its own address, and only a few of them come with a fax:
| Plan | Appeal fax | Mailing address |
|---|---|---|
| Ohio commercial (local plans, Anthem Essential Network) | 1-888-859-3046 (non-urgent; the guide gives 1-800-368-3238 for urgent appeals) | Anthem, Attn: Grievance and Appeals, P.O. Box 105568, Atlanta, GA 30348-5568 |
| Medicare Supplement (Ohio and Indiana guides); employer-group Medicare Advantage (Ohio guide) | 1-888-859-3046 | Anthem, Attn: Appeals Department, P.O. Box 105568, Atlanta, GA 30348-5568 |
| Indiana commercial | — | Anthem Blue Cross and Blue Shield, Attn: Grievance and Appeals, P.O. Box 105568, Atlanta, GA 30348-5568 |
| California commercial (Anthem Blue Cross) | — | Grievances and Appeals Department, P.O. Box 60007, Los Angeles, CA 90060-0007, with the Provider Dispute Resolution Request form |
| New York commercial | — | Payment disputes: Anthem, Attn: Claim Payment Dispute, P.O. Box 1407, Church Street Station, New York, NY 10008. Clinical appeals: Anthem, Attn: Appeals Department, P.O. Box 5063, Middletown, NY 10940 |
| Medicare Advantage, individual plans | — | Medicare Complaints, Appeals and Grievances, Attn: Medical Necessity Provider Appeals, Mailstop OH0205-A537, 4361 Irwin Simpson Road, Mason, OH 45040 |
| Indiana Medicaid (HHW, HIP, HCC) | 1-855-535-7445 (grievances and appeals) | Anthem Blue Cross and Blue Shield, Provider Disputes and Appeals, P.O. Box 61599, Virginia Beach, VA 23466 |
Medicare Advantage payment disputes that aren't about medical necessity go to the Medicare Payment Dispute Unit at the address in your state's guide: P.O. Box 1407, Church Street Station, New York, NY 10008-1407 in the New York guide, and P.O. Box 5747, Denver, CO 80217 in Nevada's.
For Anthem Medicare members
Anthem Medicare members — Medicare Advantage and Part D drug plans — send written appeals and grievances to one address and two fax lines. They are the same on Anthem's national Medicare complaints, grievances and appeals page and on every state version we checked:
- Fax: 1-888-458-1406 for medical issues, 1-888-458-1407 for pharmacy.
- Mail: Appeals and Grievances, Mailstop OH0102-B325, 4361 Irwin Simpson Rd, Mason, OH 45040.
Anthem's page calls a phone call "usually your fastest road to resolution" and lists a member line for each state and plan. If you write instead, it asks you to include your preferred contact information so a reviewer can reach you with questions — put it on the cover sheet.
For Medicaid members
These Anthem Medicaid plans publish a member fax for grievances and appeals:
| Plan | Fax | Mail or other route |
|---|---|---|
| Indiana: Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, PathWays for Aging | 1-855-516-1083 | Appeals Department, Anthem Blue Cross and Blue Shield, P.O. Box 62429, Virginia Beach, VA 23466 |
| Ohio Medicaid | 1-866-587-3316 | Email ohioga@anthem.com, or Member Services at 1-844-912-0938 |
| Virginia: Anthem HealthKeepers Plus | 1-855-832-7294 | Grievance and Appeals Department, HealthKeepers, Inc., P.O. Box 62429, Virginia Beach, VA 23464 |
In Indiana and Virginia, members have 60 days from Anthem's decision to appeal, so the date on the notice matters more than the route. Attach a copy of the notice to the fax.
For individual, family and employer plan members
Anthem Blue Cross individual and family plan members in California can fax a grievance or appeal to 1-877-551-6183, mail it to Grievances and Appeals, P.O. Box 4310, Woodland Hills, CA 91365-4310, or call 1-800-365-0609. Members have 180 calendar days from the incident or the denial letter. Anthem acknowledges a grievance within five calendar days and answers in writing within 30.
For employer plans, and for individual plans outside California, we found no general member appeal fax on Anthem's site. The denial letter says where your appeal goes, and the Member Services number on your ID card can confirm it.
Is There an Anthem Claims Fax Number?
Not one you should send claims to. Anthem's guides send claims electronically, through Availity or EDI, or on paper to a claims P.O. box that depends on the plan:
| Plan | Paper claims address |
|---|---|
| Anthem Blue Cross (California commercial and Medi-Cal) | Anthem Blue Cross, P.O. Box 60007, Los Angeles, CA 90060-0007 |
| Indiana, Ohio and Nevada commercial | Anthem, P.O. Box 105187, Atlanta, GA 30348-5187 |
| New York local plans | Anthem, Attn: Medical Claim, P.O. Box 1407, Church Street Station, New York, NY 10008-1407 |
| Ohio Medicaid | No paper claims: submit electronically, EDI payer ID 0002937 |
Indiana's guide is the one oddity: it prints a fax, 1-800-376-0247, under the claims address for its local plans without saying what that line takes. Ask Provider Services (1-800-345-4344) before faxing anything there. Members who paid a provider themselves start from Submit or Track a Claim on anthem.com.
Two kinds of claim paperwork do travel by fax:
- Records Anthem asks for. Attach them in Availity, which the guides list for medical record submission, or fax them to the number printed on Anthem's request. Our guide to faxing medical records covers the packet itself.
- SOAP notes and records for Inovalon. When the request comes from Inovalon, the Indiana, Ohio and Nevada guides point to the Inovalon ONE platform or fax 1-866-682-6680.
How to Fax Documents to Anthem
A fax to Anthem lands where it should when you pick the line for the member's state and plan, put the case details on the cover sheet and keep proof that the pages arrived. Here's the sequence in mFax, on the web or in the iPhone, Android and Windows apps:
Read the ID card, then pick the line
The card shows the member's state and whether the plan is commercial, Medicare Advantage or Medicaid. Find the number in the tables above; if Anthem's letter prints a different fax, that one wins. Enter it with the country code — +1 866 959 1537 for Medicare Advantage prior authorization.
Put the case details on the cover sheet
Give the reviewer what it takes to match the fax to a case: the member's name, ID number and date of birth, the case or claim number if you have one, and on provider requests your NPI and a callback number. Add a cover sheet in mFax and tick Confidential to print a HIPAA confidentiality notice; our HIPAA fax cover sheet guide covers what else belongs on it.
Send one complete packet
Attach the request form and every record it calls for. mFax converts Word files, photos and PDFs into fax pages and shows the page count before you send; one fax holds up to 60 pages and 100 MB.
Keep the receipt with the case
The fax shows In Progress while the pages cross — around 40 seconds a page — and Delivered once Anthem's machine confirms every page. Download receipt gives you a PDF to file with the case number.
A delivered fax proves the transmission, not that a reviewer has read it. If nothing comes back, check the case in Availity or call the provider line for the member's plan, with the receipt and page count in hand. Our guide to the fax confirmation page explains what the receipt proves.
Faxing PHI to Anthem: What HIPAA Requires
HIPAA lets a practice fax patient records to a health plan; what the practice has to add is a business associate agreement with the service that carries the fax. HIPAA applies to health plans, clearinghouses, providers that bill electronically and their business associates, as HHS's page on covered entities and business associates lays out — and a fax service handling a practice's PHI is one of those business associates.
mFax Business covers that part:
- A BAA on every plan, at no extra cost. The workspace owner opens Settings, chooses Review & sign on the Business Associate Agreement row, both sides sign electronically, and the row shows Signed with the date.
- What a privacy officer asks about: PHI stored and processed in the US, a confirmed-or-failed status on every fax, and every teammate signing in with their own login.
- A number for the answers. The practice's fax line puts every incoming fax in a shared Received list, so a reply from Anthem doesn't sit on one person's desk.
- Fax from email, too. Staff can address a message to the fax number at
send.mfax.to— the Medicare Advantage prior authorization line becomes18669591537@send.mfax.to— and attach the PDF; the address they sign in with is already authorized. The subject and body become the cover sheet, so keep patient details in the attachment. More in our email-to-fax guide.
Plans start from about $9/mo, and you build your own from the seats and pages you need. For the wider rules, see is faxing HIPAA compliant; the same steps work for other insurers, as our Aetna fax number guide shows.
Faxing your own appeal as a member? HIPAA's rules bind plans, providers and their business associates, not you sending your own records. Just need to send one fax? Open the mFax app — upload the PDF and send it in under 2 minutes, free for 7 days. On iPhone or Android, the app's scanner turns Anthem's paper letter into a fax-ready PDF.
When a Fax to Anthem Doesn't Go Through
When a fax to Anthem fails, its status names the reason — and, as mFax's support notes, most failures happen on the receiving side and a second attempt goes through:
- Recipient busy, No answer — the line was in use or didn't pick up. Busy lines are retried automatically; otherwise wait a few minutes and press Resend. Our guide to the fax busy signal explains why lines return one.
- No fax tone, Declined — the number answered, but no fax machine did. Several numbers in Anthem's guides are phone lines printed next to the fax lines; check yours against the tables.
- Invalid number, Number not in service — re-check the digits and the +1.
- File too large — the packet is over 60 pages or 100 MB. Split it into two faxes and mark each cover sheet "1 of 2" and "2 of 2".
Fax Anthem from Your Practice's Own Number
mFax Business gives your practice a fax number for Anthem's answers, HIPAA-compliant faxing with a signed BAA, and a delivery receipt for every packet — on web, mobile and desktop. Build your own plan from about $9/mo, and cancel anytime.
