Quick Answer: Wellcare has no single fax number — its 2026 forms route each request by type and state. Outpatient authorization forms list a line per state (1-877-892-8216 for Florida Medicare-only members, 1-888-361-5684 for states without their own line), inpatient requests go to 1-855-776-9464, Part D coverage requests to 1-866-388-1767, Part D appeals to 1-866-388-1766, and medical appeals and claim disputes to 1-866-201-0657. Claims can't be faxed at all. A practice that faxes Wellcare every week can send from its own number, with a signed BAA, on mFax Business.
Search for a Wellcare fax number and the PDFs you find disagree with each other, because they really do. Wellcare's 2026 authorization forms print one set of lines, its state Quick Reference Guides print another, and older guides still linked from some state pages print a third. In April 2026 Wellcare reminded providers to keep sending faxes to its toll-free numbers and to confirm the right ones on wellcare.com/providers or the provider portal. A Wellcare reminder dated April 27 adds that "toll-free fax numbers are listed on all applicable forms."
Below is every Wellcare fax number we could confirm in Wellcare's own forms and provider guides, read on October 26, 2026, with the document each one comes from. They cover Wellcare's Medicare Advantage and prescription drug (PDP) plans. You'll also find what Wellcare won't take by fax, what belongs on the cover sheet, and how to fax patient records with a business associate agreement in place.
Wellcare Fax Numbers at a Glance
The right Wellcare fax number depends on what you're sending and, for authorizations, on the member's state. Find your row, then read its section below for the conditions that come with it.
| What you're sending | Wellcare fax number | Where it's printed |
|---|---|---|
| Outpatient, DME, home health, hospice, therapy, surgery or transportation authorization | The member's state line — e.g. 1-877-892-8216 (Florida, Medicare only); 1-888-361-5684 for states without their own | 2026 authorization forms |
| Inpatient authorization (admission, observation, SNF, rehab, LTACH) | 1-855-776-9464 | Inpatient Authorization Request Form |
| Part B drug authorization (drugs the office or facility administers) | 1-888-871-0564 | Medical Drug Authorization Request |
| Behavioral health authorization | The member's state line — see the table below | 2026 behavioral health forms |
| Part D coverage determination (drug prior authorization, exceptions) | 1-866-388-1767 (Medicare Advantage in California: 1-877-277-1809) | Coverage determination form |
| Part D appeal (redetermination) | 1-866-388-1766 | Redetermination form |
| Medical appeal, reconsideration or claim payment dispute | 1-866-201-0657 | 2026 claims and appeals guides |
| Member grievance (complaint) | 1-866-388-1769 | 2026 appeals and grievances guide |
| Overpayment administrative review | 1-877-277-1805 | Claims, Disputes & Recovery guide |
| Care or disease management referral | 1-866-287-3286 | State Quick Reference Guides |
| Claims | No fax — electronic (payer ID 14163 or 68069) or mail | Claims, Disputes & Recovery guide |
The form or letter in your hand beats this table
If Wellcare sent you a denial notice, an authorization response or a records request, use the fax number printed on it. Check the member's ID card too: some states sell Wellcare plans through a local Centene health plan — Wellcare by Allwell in Texas, Wellcare by Fidelis Care in New York — with their own contacts, and WellCare's Medicaid plans in Kentucky and North Carolina have websites of their own.
Wellcare Prior Authorization Fax Numbers
The Wellcare prior authorization fax number is the one printed on the form you're sending. Wellcare's authorizations page tells providers to complete the Medicare authorization form and fax it "to the fax number listed on the form," and the forms revised in February 2026 route by the member's state.
The secure provider portal remains Wellcare's first choice — "the fastest and most efficient way to request an authorization," in the page's words. Fax is the alternative for standard requests, and urgent ones go by phone.
Outpatient, DME, home health and other services
Seven of Wellcare's 2026 authorization forms — Outpatient, DME, Home Health, Hospice, Skilled Therapy (OT/PT/ST), Surgery and Transportation — print the same "Medicare Fax Lines" at the bottom, as on the Outpatient Authorization Request Form:
| Member's state | Fax the form to |
|---|---|
| Connecticut | 1-866-455-6529 |
| Florida, Medicare only | 1-877-892-8216 |
| Georgia, Medicare only | 1-877-892-8213 |
| Florida or Georgia, dual-eligible | 1-877-277-1820 |
| Illinois | 1-877-899-2044 |
| Kentucky | 1-888-361-5684 |
| New Jersey | 1-877-892-8221 |
| New York | 1-877-892-8214 |
| Texas | 1-877-894-2034 |
| All other states | 1-888-361-5684 |
Wellcare's authorizations page lists what a request generally needs: the member's name and ID number, the treating provider's ID and NPI, the facility's ID and NPI where one is involved, the provider or facility fax number, dates of service, diagnosis codes and CPT codes. Don't skip the fax field — Wellcare warns that its fax response can arrive before the decision shows in the portal.
Its 2026 Prior Authorization Submission Checklist covers the clinical side: recent notes (generally from the last 90 days), a signed order or prescription, the plan of care where one is required, relevant imaging, labs or specialist notes, and face-to-face documentation when required. Label every page with the member's name and date of birth, remove duplicates, and send "all required documents submitted together in one packet."
Inpatient and Part B drug requests
Two request types use a single line for every state:
- Inpatient — observation, admission, LTACH, skilled nursing or sub-acute rehab, inpatient rehab. The Inpatient Authorization Request Form says "Please fax completed form to 1-855-776-9464." Wellcare's guides require notification of an inpatient hospital admission by the next business day, normal maternity deliveries excepted.
- Part B drugs — drugs a physician or facility supplies and administers. The Medical Drug Authorization Request goes to 1-888-871-0564, the line the state guides call "Pharmacy Medical Requests."
Part D drugs, the ones a member fills at a pharmacy, follow a different process with different numbers — see the pharmacy section below.
Why the Quick Reference Guide shows another number
Wellcare's state Quick Reference Guides for 2026 and 2027 print one "Medical Fax" for prior authorization in most states, 1-877-277-1805, instead of the forms' state lines. When you're sending one of the forms above, the number on the form is the one Wellcare's authorizations page tells you to use.
Two more numbers circulate. Arizona's and Iowa's authorization pages still link 2025 guides that list 1-833-562-7172 as the medical fax. And Illinois's guide for January 2027 has already swapped the general line for the forms' pair: inpatient 1-855-776-9464, outpatient 1-877-899-2044.
Urgent requests go by phone
Don't fax an expedited request. Wellcare's forms send it to the phone: if the standard time frame could seriously jeopardize the member's life, health or ability to regain maximum function, call Provider Services — 1-855-538-0454 in most states (New York: 1-844-429-5825; Hawaii: 1-888-505-1201). Wellcare decides expedited requests within 72 hours, and its guides add that "phone authorizations must be followed by a fax submission of clinical information."
When a vendor handles the request
Some services aren't authorized by Wellcare directly. Florida's 2026 Prior Authorization Guide, for example, sends them to these vendors:
| Service (Florida) | Who authorizes it | Contact |
|---|---|---|
| Advanced imaging, cardiology, orthopedic and spinal surgery | Evolent | 1-800-424-5388 |
| Medical oncology, radiation therapy | Evolent | 1-888-999-7713 |
| Sleep studies, PT/OT, pain management, molecular and genetic lab tests | eviCore | 1-888-333-8641 |
| Skilled nursing, inpatient rehab and LTACH admissions | CareCentrix | 1-888-571-6028 |
| Home health | CareCentrix | Fax 1-877-246-8088 |
| DME purchases and rentals | Coastal Care Services | Fax 1-855-481-0606 |
Vendors differ from state to state. Open the Prior Authorization Guide on your state's Wellcare authorizations page before you fax a DME, home health or post-acute request.
Wellcare Behavioral Health Fax Numbers
Wellcare's 2026 behavioral health request forms route by state and by level of care. The inpatient-level forms (inpatient, sub-acute and crisis stabilization; detox and substance abuse rehab) and the outpatient-level forms (routine outpatient, IOP, PHP, ECT, and psychological or neuropsychological testing) each print their own line:
| Member's state | Inpatient-level requests | Outpatient-level requests |
|---|---|---|
| Florida | 1-855-710-0167 | 1-855-710-0168 |
| Georgia | Medicare only 1-877-892-8213; dual-eligible 1-855-292-0233 | Same as inpatient |
| Hawaii | 1-888-890-8219 | 1-888-881-8225 |
| Kentucky | 1-888-365-5615 | 1-888-365-5676 |
| New York | 1-855-713-0588 | 1-855-713-0589 |
| Texas | 1-855-671-0258 | 1-855-671-0259 |
| Connecticut, Maine, North Carolina | 1-888-365-3233 | 1-888-365-5607 |
| Arkansas, Louisiana, Mississippi, South Carolina, Tennessee | 1-855-710-0159 | 1-855-710-0160 |
| Illinois, Michigan, Missouri, Washington | 1-855-713-0592 | 1-855-713-0593 |
The Quick Reference Guides give one pair for most states — 1-855-710-0159 for inpatient and 1-855-710-0160 for outpatient — so the same rule applies: the form wins. New Jersey isn't on the forms' list; confirm its line with Provider Services. Florida's guide adds that psychological testing requests go by fax, while other behavioral health levels of care can be submitted online.
Wellcare Pharmacy Fax Numbers for Part D
Part D drug requests go to Wellcare's pharmacy unit on two lines: 1-866-388-1767 for coverage determinations and 1-866-388-1766 for appeals. The member, the prescriber or an authorized representative can file either one.
Coverage determinations (drug prior authorization)
Wellcare's Request for Medicare Drug Coverage Determination covers prior authorization, formulary and tiering exceptions, quantity limits, and repayment for a drug already bought. Send it with the supporting information to:
- 1-866-388-1767 — every prescription drug plan (PDP), and Medicare Advantage plans outside California
- 1-877-277-1809 — Medicare Advantage plans in California
- or by mail to WellCare Health Plans, Pharmacy – Coverage Determinations, P.O. Box 31397, Tampa, FL 33631-3397
Exception requests need a supporting statement from the prescriber, on pages 3 and 4 of the form. A standard decision takes up to 72 hours; if the prescriber says waiting could seriously harm the member's health, Wellcare decides within 24 hours. Prescribers can also file electronically through CoverMyMeds, the ePA route the state guides list. Anyone other than the member or the prescriber attaches Form CMS-1696, the Appointment of Representative, or an equivalent.
Part D appeals (redeterminations)
If Wellcare denies the drug, the appeal — a redetermination — goes to 1-866-388-1766, or by mail to Wellcare, Attn: Medicare Pharmacy Appeals, P.O. Box 31383, Tampa, FL 33631-3383. Wellcare's drug appeal page and form allow 65 days from the date of the Notice of Denial of Medicare Prescription Drug Coverage. The pharmacy box in the provider guides still says 60 days, so a prescriber appealing for a patient shouldn't wait for the last week.
A standard appeal is decided within 7 days and an expedited one within 72 hours. Expedited appeals are requested by phone, at the Member Services number on the ID card, and can't be used to get paid back for a drug already received. Appeals can also be filed online at wellcare.com.
Where Express Scripts fits
Express Scripts Pharmacy is Wellcare's preferred mail-order service — 1-833-750-0201, 24 hours a day — but Wellcare makes the coverage decisions. Send a Wellcare member's coverage request or appeal to the two Wellcare lines above, not to Express Scripts' own Medicare fax numbers.
For a new mail-order prescription, Wellcare's Express Scripts order form says e-prescribing is fastest and that doctors should call 1-888-327-9791 for faxing instructions; faxes are accepted only from a doctor's office. Our Express Scripts fax number guide covers that side.
Is There a Wellcare Claims Fax Number?
No. Wellcare's Claims, Disputes & Recovery Guide says it outright: "Wellcare does not accept handwritten, faxed or replicated claim forms," and it doesn't take CDs, DVDs or USB drives either. Claims go electronically — through Availity, Wellcare's preferred clearinghouse, or your existing one, or as free direct data entry on Availity Essentials — or on paper by mail. The filing limit is 180 days from the date of service unless your contract says otherwise.
| Member's state | Payer ID | Paper claims (printed CMS-1500 or UB-04) |
|---|---|---|
| AR, CT, FL, GA, HI, IL, KY, LA, ME, MI, MO, MS, NC, NJ, NY, SC, TN, TX, WA | 14163 | Wellcare, Attn: Claims Department, P.O. Box 31372, Tampa, FL 33631-3372 |
| AZ, DE, IA, IN, KS, NE, NV, OH, OK, PA, WI | 68069 | Wellcare, Attn: Claims Department, P.O. Box 3060, Farmington, MO 63640-3060 |
Disputes, reconsiderations and appeals
Disputes are where fax comes back in. Wellcare's claims and appeals guides for 2026 and 2027 send claim payment disputes, medical appeals and reconsiderations, with supporting documentation, to the Appeals Department: fax 1-866-201-0657, or mail to Attn: Appeals Department, P.O. Box 31368, Tampa, FL 33631-3368 (P.O. Box 3060, Farmington, MO 63640-3060 for the second group of states). The provider portal takes them too.
| Request | Who files | Deadline |
|---|---|---|
| Claim payment dispute (untimely filing, unlisted or non-covered codes) | Provider | 90 calendar days from the date on the EOP |
| Reconsideration of a denial for no authorization, services beyond it, missing documentation or late notification | Participating provider | 90 calendar days from the denial |
| Appeal of an adverse determination | Non-participating provider | 65 calendar days from the notice, with a signed Waiver of Liability |
Name the code or service you're disputing, and add a summary, the relevant medical records and the member's details. Both guides ask you to leave out the image of the claim. A billing company appealing for a non-participating provider also needs an Appointment of Representative form.
Older Wellcare documents — Texas's claims guide approved in September 2025, and a Michigan provider bulletin — send claim payment disputes to P.O. Box 31370 and fax 1-877-277-1808 instead. If your denial letter names a different fax number, follow the letter.
Overpayments and records Wellcare asks for
Three more kinds of claim paperwork go by fax:
- Overpayment reviews. If you disagree with a Wellcare overpayment notice, request an administrative review within 45 days of the recovery letter: fax 1-877-277-1805, or mail Attn: CCU Recovery, P.O. Box 31658, Tampa, FL 33631-3658. Wellcare won't consider documents added after the first request and issues a final determination within 30 days.
- Payment-integrity reviews. Wellcare's claims guide sends first-level appeals of CPI edits to Optum (fax 1-267-687-0994, or secure upload as Optum's records letter describes) and reviews of DN227, DN228 and RV213 denials to Cotiviti (fax 1-203-202-6607).
- Medical records. There's no general Wellcare "medical records fax number." Send requested records to the fax or upload address on the request letter itself; our guide to faxing medical records covers the packet.
Wellcare Appeal and Grievance Fax Numbers for Members
Members appeal to the same Wellcare departments as providers. Wellcare's appeals and grievances guide puts members alongside non-participating providers for medical appeals:
| Member request | Fax | |
|---|---|---|
| Appeal of a denied medical service or Part B drug | 1-866-201-0657 | Wellcare, Attn: Appeals Department, P.O. Box 31368, Tampa, FL 33631-3368 |
| Appeal of a denied Part D drug (redetermination) | 1-866-388-1766 | Wellcare, Attn: Medicare Pharmacy Appeals, P.O. Box 31383, Tampa, FL 33631-3383 |
| Grievance (complaint) | 1-866-388-1769 | Wellcare, Attn: Grievance Department, P.O. Box 31384, Tampa, FL 33631-3384 |
The notice that denied the service names where its appeal goes, and that notice wins. In the second group of claims states (AZ, DE, IA, IN, KS, NE, NV, OH, OK, PA, WI), medical appeals go by mail to P.O. Box 3060, Farmington, MO 63640-3060 and grievances to P.O. Box 10450, Van Nuys, CA 91410-0450; the fax numbers are the same.
A grievance can also be made by phone to Member Services or through Wellcare's Contact Us page. A participating physician who is treating the member can appeal a denied service with the member's written consent; anyone else, a family member included, needs an Appointment of Representative form signed by the member and the representative.
Wellcare Phone Numbers That Aren't Fax Lines
Some numbers that turn up in Wellcare fax searches are phone lines. 1-855-538-0454 is the most common: it's Wellcare's Provider Services line (IVR) for Medicare plans in most states, not a fax.
| Line | Number | Notes |
|---|---|---|
| Provider Services, Medicare plans | 1-855-538-0454 | Most states; New York 1-844-429-5825, Hawaii 1-888-505-1201; D-SNP plans have their own line |
| Provider Services, prescription drug plans | 1-855-538-0453 | One digit off the line above |
| Member Services, Medicare plans | Number on the member ID card | 1-833-444-9088 in Florida, New York and Texas |
| Member Services, prescription drug plans | 1-888-550-5252 | TTY 711 |
| Express Scripts mail order | 1-833-750-0201 | 24 hours a day, 7 days a week |
Member Services answers from 8 a.m. to 8 p.m., seven days a week from October 1 to March 31 and Monday to Friday from April 1 to September 30. Wellcare's contact page warns that some third parties use phone numbers that closely resemble its own, and asks members to call only the numbers on wellcare.com or the ID card. The same caution applies to fax numbers copied from directory sites.
How to Fax Documents to Wellcare
A fax to Wellcare reaches the right department when you take the number from the form, put Wellcare's identifiers on the cover sheet and keep proof that the pages arrived. Here's the sequence in mFax, on the web or in the iPhone, Android and Windows apps:
Take the number from the form
Use the line printed on the Wellcare form or letter you're answering; the tables above are the fallback. Enter it with the country code — +1 888 361 5684 for an outpatient request from a state without its own line.
Put Wellcare's identifiers on the cover sheet
Wellcare's forms ask for the member's name, Wellcare ID and date of birth, your NPI or Tax ID, and your fax number; add the authorization or claim number when you have one. Add a cover sheet in mFax and tick Confidential to print a HIPAA confidentiality notice. Our HIPAA fax cover sheet guide covers what else belongs on it.
Send one complete packet
Attach the form and every record it needs — Wellcare asks for one packet, with duplicate pages removed. mFax converts Word files, photos and PDFs into fax pages and shows the page count before you send; one fax holds up to 60 pages and 100 MB.
Keep the receipt with the case
The fax shows In Progress while the pages cross — around 40 seconds a page — and Delivered once Wellcare's machine confirms every page. Download receipt gives you a PDF to file with the case, which settles the question if anyone says nothing arrived.
A delivered fax proves the transmission, not that a reviewer has picked it up. Providers can check status in the portal or with Provider Services, and members at the number on their ID card; have the date, the page count and the receipt at hand. Our guide to the fax confirmation page explains what that proof shows. Faxing other payers too? Our Aetna fax number guide does the same job for Aetna's lines.
Faxing PHI to Wellcare: What HIPAA Requires
HIPAA lets a practice fax patient records to a health plan; what the practice adds is a business associate agreement with the service that carries the fax. HIPAA applies to health plans, clearinghouses, providers that bill electronically and their business associates, as HHS lays out on its page about covered entities and business associates — and a fax service handling a practice's PHI is one of those business associates.
mFax Business covers that part:
- A BAA on every plan, at no extra cost. The workspace owner opens Settings, chooses Review & sign on the Business Associate Agreement row, both sides sign electronically, and the row shows Signed with the date.
- What a privacy officer asks about: PHI stored and processed in the US, a confirmed-or-failed status on every fax, and every teammate signing in with their own login.
- A fax number for Wellcare's answers. Every Wellcare authorization form makes the provider's fax number a required field, and Wellcare's fax response can arrive before the decision shows online. The practice's fax line drops those replies into a shared Received list.
- Fax from email, too. Staff can address a message to the fax number at
send.mfax.to— the "all other states" outpatient line becomes18883615684@send.mfax.to— and attach the PDF; the address they sign in with is already authorized. The subject and body become the cover sheet, so keep patient details in the attachment. More in our email-to-fax guide.
Plans start from about $9/mo, and you build your own from the seats and pages you need. For the wider rules, see is faxing HIPAA compliant.
Faxing your own appeal as a member? HIPAA's rules bind plans, providers and their business associates, not you sending your own records. Just need to send one fax? Open the mFax app — upload the PDF and send it in under 2 minutes, free for 7 days. On iPhone or Android, the app's scanner turns Wellcare's paper notice into a fax-ready PDF.
When a Fax to Wellcare Doesn't Go Through
When a fax to Wellcare fails, its status names the reason — and, as mFax's support notes, most failures happen on the receiving side and a second attempt goes through:
- Recipient busy, No answer — the line was in use or didn't pick up. Busy lines are retried automatically; otherwise wait a few minutes and press Resend. Our guide to the fax busy signal explains why lines return one.
- No fax tone, Declined — the number answered, but no fax machine did. You have most likely dialed a phone line, such as Provider Services; check the number against the form.
- Invalid number, Number not in service — re-check the digits and the +1.
- File too large — the packet is over 60 pages or 100 MB. Split it into two faxes and mark the cover sheets "1 of 2" and "2 of 2".
Wellcare also posts maintenance notices on wellcare.com warning that its systems and fax may be unavailable during the window. If a fax fails during one, resend it once the window ends.
Fax Wellcare from Your Practice's Own Number
mFax Business gives your practice a fax number for Wellcare's answers, HIPAA-compliant faxing with a signed BAA, and a delivery receipt for every packet — on web, mobile and desktop. Build your own plan from about $9/mo, and cancel anytime.
