Express Scripts Fax Number: Prior Auth & Prescriptions

There is no single Express Scripts fax number: new prescriptions, prior authorizations, appeals, TRICARE orders and Accredo referrals each have their own line. Here is every number from Express Scripts' own forms, who is allowed to fax what, and the form rules that hold a fax up.

Express Scripts Fax Number: Prior Auth & Prescriptions

By David Thompson · Published July 25, 2026 · 10 min read

Quick answer: Express Scripts has no single fax number. The line depends on what you are sending: prescribers fax new home-delivery prescriptions to 1-800-837-0959 and prior authorization requests to 1-877-251-5896, while members fax benefit coverage requests to 1-877-328-9660. A practice without a fax machine can send all of them from a browser, phone or PC with mFax Business: HIPAA-compliant faxing with a signed BAA, under the practice's own fax number.

Search for the Express Scripts fax number and Google returns a stack of PDFs, each printing a different line. That is by design. Express Scripts, the pharmacy benefit manager that is now part of Cigna's Evernorth, sends new prescriptions, prior authorizations, appeals, TRICARE orders and specialty referrals to separate departments. Its Medicare page even gives each department its own PO box.

Every number below was read from Express Scripts', Evernorth's or Accredo's own forms and pages on October 26, 2026, with a link to each source. If a form in your hand prints a different number, the form wins. The general rules for faxed prescriptions are in our guide to faxing a prescription to a pharmacy. This page is the Express Scripts directory.

Express Scripts Fax Numbers at a Glance

The right Express Scripts fax number is the one tied to your request: prescriptions, prior authorizations, coverage reviews and appeals each go to a different line.

Two numbers appear twice, and that is not a typo. 1-877-251-5896 takes both commercial prior authorizations and Medicare coverage determinations. 1-877-328-9660 takes both commercial benefit coverage requests and Medicare administrative reviews.

Patients can't fax prescriptions

Express Scripts takes faxed prescriptions only from the prescriber's office. Its home delivery sheet says it plainly: "Only providers can fax prescriptions." The TRICARE prescription form adds: "not your home or work." A patient can ask the doctor to e-prescribe or fax. What a patient can fax is coverage paperwork, covered in the appeals section below.

How to Fax a New Prescription to Express Scripts Pharmacy

Prescribers fax new home-delivery prescriptions to 1-800-837-0959, on Express Scripts' New Prescription Fax Form. The form fits the prescription and everything Express Scripts needs to ship it onto a single page. Questions about the form go to 1-888-327-9791.

The form spells out its own rules, and it warns that "incomplete forms will cause a delay in processing":

  • Prescriber details: name, NPI, the office fax number, and a DEA number for Schedule III–V drugs only.
  • Member details: the card member number with every character, the patient's date of birth and a ship-to address.
  • Medication count: write how many medications are on the fax.
  • Signature: "Stamps are not accepted. Signature required."
  • Brand name: the prescriber must handwrite "brand necessary" or "brand medically necessary" for a brand to be dispensed.
  • No cover sheet: "Do not fax with a cover sheet." A second version of the form (form 34202) says "Fax this page ONLY."
  • No Schedule II: the form is "Not for CII prescriptions."
  • Send from the right line: "Fax from the prescriber's secure fax line."

Express Scripts still points prescribers to e-prescribing first. Its current home delivery brochure says an electronic prescription "is the quickest way to get your medication." In e-prescribing software the pharmacy is listed as Express Scripts Home Delivery.

If you are the patient, you have three routes, none of them a fax from home:

  • Ask your doctor's office to e-prescribe a 3-month supply to Express Scripts Home Delivery, or to fax the form above.
  • Log in at express-scripts.com/rx and choose Request an Rx.
  • Call the number on the back of your prescription ID card, and Express Scripts contacts your doctor for you.

A paper prescription you already hold can go by mail, with the Home Delivery Order Form from your Express Scripts account. Refill requests and transfers at a retail pharmacy work differently; our guide on how to fax a pharmacy covers those.

Express Scripts Prior Authorization Fax Number

The Express Scripts prior authorization fax number is 1-877-251-5896, the line printed on its prior authorization general request form. Fax is Express Scripts' fallback, though, not its first choice.

Express Scripts' prior authorization page now lives on Evernorth's site: express-scripts.com/pa redirects to Evernorth's Prior Authorization Resources. It lists the routes in this order:

  1. Electronic prior authorization (ePA) through CoverMyMeds, Surescripts or EviCore by Evernorth. Express Scripts says more than half of the prior authorizations it receives each month already arrive this way.
  2. Phone: call 800-753-2851 for a verbal request.
  3. Fax: send the general request form, or your state's form, to the number printed on it.

The general request form carries a date of 08.22.2011, but it is the version the page links today. It asks for the patient's name, ID and date of birth, and the prescriber's DEA or NPI, phone and fax. It also asks for the diagnosis with its ICD code, the drug, strength, quantity and days' supply, and the therapies already tried with why they failed. For an urgent request, the form prints 1-800-417-8164.

Several states have their own form. Evernorth links forms for Arizona, California, Colorado, Illinois, Iowa, Kansas, Louisiana, Massachusetts, Nebraska, New Hampshire, New York, Oklahoma and Texas, plus one shared by Arkansas, Michigan, Oregon and Vermont. The Iowa, Oklahoma and four-state forms print the same 1-877-251-5896. One exception: for patients insured through Blue Cross Blue Shield of Louisiana, the page sends prescribers to 800-842-2015 and a separate form.

Express Scripts Appeal and Coverage Review Fax Numbers

Unlike prescriptions, appeals and coverage requests can come from the member, the prescriber or an authorized representative. Which fax number you use depends on whether the plan is commercial or Medicare Part D.

Commercial plans

The member-side form is the Benefit Coverage Request Form. Fax it to 1-877-328-9660, or mail it to Express Scripts, Attn: Benefit Coverage Review Department, PO Box 66587, St. Louis, MO 63166-6587. The form's own examples of what it handles:

  • a lower copay for a brand-name drug, or a tiering exception
  • payment for a drug that isn't covered, or for a larger quantity
  • an appeal of a denied reimbursement

For plans under ERISA, the form says the written decision comes within 15 days of receipt, or within 30 days for reimbursement of a drug you already received.

A clinical appeal, after a prior authorization denial, works differently. Express Scripts' PA page says its general request form also serves "to make an appeal for us to reconsider a coverage decision." Your denial letter names the exact appeal address and fax for your plan, and the letter wins. Some plans send clinical appeals to Express Scripts' Clinical Appeals Department, PO Box 66588, fax 1-877-852-4070. Missouri's public employee plan, MCHCP, lists exactly that for its Express Scripts members.

Medicare Part D plans

Evernorth's Medicare coverage page gives each request type its own fax, mailbox and phone:

The deadlines come from Express Scripts' own Medicare forms:

  • Coverage determination: a standard decision within 72 hours. If the prescriber says waiting could seriously harm your health, the decision comes within 24 hours.
  • Redetermination (appeal): ask within 65 days of the date on the denial notice, and include a copy of it. A standard decision comes within 7 days, or within 72 hours if expedited.
  • Someone filing for you must attach Form CMS-1696 (Appointment of Representative) or an equivalent document.

Not every Medicare plan that works with Express Scripts sends coverage requests here. Wellcare, for one, uses Express Scripts Pharmacy for mail order but makes its own Part D decisions: its members' coverage determinations and appeals go to Wellcare's lines, listed in our Wellcare fax number guide.

TRICARE Pharmacy Fax Numbers (Express Scripts)

Express Scripts runs the TRICARE pharmacy program, and its TRICARE site prints two different prescription fax numbers. Know which page each comes from before you send.

Express Scripts TRICARE pageWho it addressesPrescription fax
Prescribers pagePrescribers(877) 250-8255
Member FAQ and the TRICARE new prescription form (last updated 03/07/2018)Members and their doctors(877) 895-1900 in the U.S.; (602) 586-3911 from abroad, U.S.-licensed prescribers only

Both are Express Scripts' own pages, read on the same day. A prescriber can use the number on the page written for prescribers. Anyone unsure can call the provider line, (877) 363-1303, before sending.

The rest of the TRICARE prescriber contacts:

  • Call-in prescriptions: (877) 283-3858
  • Prior authorization: CoverMyMeds for network providers, Surescripts for military treatment facility providers, or by phone at (866) 684-4488
  • E-prescribe to: Express Scripts Home Delivery, 4600 North Hanley Road, St. Louis, MO 63134
  • Mail: Express Scripts, PO Box 52150, Phoenix, AZ 85072-9954

The TRICARE form repeats the core rules: faxes "must be sent from a doctor's office, not your home or work," and Schedule II prescriptions can't be faxed.

Accredo Specialty Pharmacy Fax Numbers

Specialty drugs go to Accredo, which its own forms call "an Express Scripts Company." Accredo uses several fax lines, and the one on the form in your hand is the one to use.

  • General referral (Prescription & Enrollment Form): fax all pages to 888-302-1028. The team's phone is 844-516-3319.
  • Drug-specific forms print their own team fax. The Dupixent form, for example, sends to 866-531-1025. Pick the drug's form on Accredo's referral forms page.
  • General patient fax: 877-329-4605, which Accredo's contact page reserves for referrals, prior authorizations, clinical notes and replies to Accredo forms sent to the patient. Patients in Hawaii use 808-650-6487.

Prescribers registered on Accredo's MAP portal (My Accredo Patients) can also send a prescription through MAP Fax Rx. The fax then shows up in the portal's Message Center. Accredo's phone line is (877) 222-7336, around the clock.

How to Fax Express Scripts Without a Fax Machine

You don't need a fax machine to reach any Express Scripts fax number. An online fax service dials the same line and gets the same delivery confirmation back. The second version of Express Scripts' New Prescription form says "the printed fax confirmation is proof of receipt", so keep that confirmation.

Here is the send flow in mFax:

1

Get the right form

Download it from the page the number came from. Member forms such as the Benefit Coverage Request Form can be filled in on screen in our free PDF editor. A prescription needs the prescriber's own signature (Express Scripts doesn't accept stamps): sign the printed form, then scan it with the camera in the mFax iPhone or Android app.

2

Enter the fax number

Choose New Fax and type the number with its country code, for example +1 800 837 0959 for a home-delivery prescription.

3

Attach the form and skip the cover sheet

In mFax a cover sheet is optional, so for prescription forms don't add one. Express Scripts asks for the form page only. For other paperwork, if you do add one, tick Confidential to print a HIPAA confidentiality notice. Our medical fax cover sheet guide explains what goes on it.

4

Send and keep the receipt

The fax shows Delivered once the receiving machine confirms every page, and Download receipt gives you a PDF. That proves the transmission, not that Express Scripts has processed the request. Our guide to the fax confirmation page covers what it shows.

For a practice or clinic, mFax Business is built for this traffic. It gives you a team fax line in the practice's name, and every Business plan includes a signed BAA at no extra cost. PHI is stored and processed in the US, every fax ends in a confirmed or failed status, and each teammate signs in with their own login. It runs on web, iPhone, Android and Windows, and plans start from about $9/mo, with seats and pages you set yourself. Our explainer on the BAA for a fax service covers why a practice needs that agreement before PHI goes out.

The forms ask for your fax number, and the TRICARE form and the second New Prescription form call it the Return Fax, so give a line that can receive. Every mFax plan includes a number for receiving faxes, and on a Business workspace it is the shared team fax line.

For a member faxing their own appeal or coverage request, a one-off fax doesn't need a business plan. Open mFax in your browser, upload the form and send it free with the 7-day trial.

Why Express Scripts Faxes Get Delayed or Rejected

Most delays trace back to rules printed on Express Scripts' own forms. Check these before you press Send:

  • Wrong line for the request. A prior authorization sent to the prescription fax, or a Medicare appeal sent to the coverage determination line, lands in the wrong department. Match the request to the table at the top.
  • A cover sheet on a prescription form. The forms say not to use one.
  • A Schedule II prescription. It can't be faxed at all.
  • A signature stamp. Express Scripts requires a real signature.
  • Faxed from the wrong place. Prescriptions must come from the prescriber's office, not a patient's home or workplace.
  • Missing IDs. The member number needs every character, the NPI is required, and Schedule III–V drugs need a DEA number. "Incomplete forms will cause a delay in processing."
  • No "brand medically necessary." Without that handwritten line, the brand-name drug isn't dispensed.
  • Medicare paperwork gaps. An appeal needs a copy of the denial notice, and a representative needs Form CMS-1696.

If the fax itself fails, the problem is the call, not the form. A busy line needs a resend, while "no fax tone" means the number didn't answer as a fax line. Our list of fixes for a fax that won't go through covers the rest. Before PHI leaves the office, our checklist for faxing PHI safely is worth a read.
